Medical Insurance Participation

Meeting the needs of our community is a top priority for Professional Eye Associates, so we are proud to participate with many of the medical insurance plans offered in our region.  These plans include:

  • Medicare
  • Medicare Advantage and supplement plans
  • Medicaid
  • Alliant
  • CIGNA
  • Certain United Healthcare Plans
  • Certain BCBS plans
  • And many others.

If your employers' plan is not on this list, please give us a call. 

As a courtesy to our patients, we will gladly submit claims to these insurance companies.  Please note that patients are responsible for payment of all treatment not covered by their insurance plan.

 

Vision Insurance Participation

We are also proud to participate with the following vision plans:

  • Certain EyeMed plans
  • J&J Direct Reimbursement
  • Beaulieu S & S Healthcare strategies
  • Certain Simple, Inc. plans

If your vision plan is not on this list, please give us a call.
 

Payments

Payment is due at the time services are rendered.  This includes insurance co-pays, annual deductible, or any portion of the visit not covered by insurance.  We accept cash, check, VISA, MasterCard and Discover. 

If an outstanding balance should result after your insurance company processes your claim, you will be sent a statement of the balance due.  Payment is due in full by the date posted on the statement.

If a credit balance should result after your insurance company processes your claim and after you have already paid the balance, we will promptly send you a refund.

 

Finance Options

It is our mission to provide you with affordable, high-quality care that fits your budget! We offer a variety of payment options to meet your needs. Our office accepts payments by cash, checks, VISA, MasterCard and Discover. We also offer low- and no-interest financing options through CareCredit. If you have any questions about financing or payments, ask us! We will thoroughly explain your choices and work hard to accommodate your needs.

Late Arrivals, Cancellations, and Missed Appointments

We ask first time patients to arrive at our office 15 minutes prior to the scheduled appointment time to complete the registration process.  Existing patients should arrive 5 minutes prior to their scheduled appointment to check-in.  While we make every effort to accommodate our patients, late arrivals may result in longer wait times or you may reschedule your appointment.

If you are unable to keep your appointment, please give us a 24 hour notice so that we can accommodate other patients.   Any patient who does not notify us of their inability to keep an appointment will incur a $20.00 rescheduling fee. The practice reserves the right to dismiss patients for repeated no-shows.  By scheduling an appointment you are agreeing to our cancellation policy.

 

Children

For the safety and security of our young patients, it is our policy that all children under the age of 18 must be accompanied by a parent or legal guardian. If another adult must bring the child to the appointment, please provide written permission for this adult to accompany the patient and to make any necessary medical decisions on behalf of the patient.
