Medical Insurance Participation

Meeting the needs of our community is a top priority for Professional Eye Associates, so we are proud to participate with many of the medical insurance plans offered in our region. These plans include:

  • Medicare
  • Medicare Advantage and supplement plans
  • Medicaid
  • Alliant
  • CIGNA
  • Certain United Healthcare Plans
  • Certain BCBS plans
  • And many others

If your employer's plan is not on this list, please give us a call.

Since insurance coverage differs, for quickest service come to us with an understanding of the eye care coverage your plan provides. As a courtesy to our patients, we will gladly submit claims to these insurance companies. Please note that patients are responsible for payment of all treatments not covered by their insurance plan.

Vision Insurance Participation

We are also proud to participate with the following vision plans:

  • Certain EyeMed plans
  • Most Simple, Inc. plans
  • VSP

If your vision plan is not on this list, please give us a call.

Payments

Payment is due at the time services are rendered. This includes insurance co-pays, annual deductible, or any portion of the visit not covered by insurance. We accept cash, checks, American Express, VISA, MasterCard, and Discover.

If an outstanding balance should result after your insurance company processes your claim, you will be sent a statement of the balance due. Payment is due in full by the date posted on the statement.

If a credit balance should result after your insurance company processes your claim, and after you have already paid the balance, we will promptly send you a refund.

Finance Options

We strive to provide you with affordable, high-quality care that fits your budget. We offer a variety of payment options to meet your needs. Our office accepts payments by cash, checks, American Express, VISA, MasterCard and Discover. We also offer low- and no-interest financing options through CareCredit. If you have any questions about financing or payments, please ask us. We will thoroughly explain your choices and work hard to accommodate your needs.

Late Arrivals, Cancellations and Missed Appointments

  • First-Time Patients: Please arrive at our office 15 minutes prior to your scheduled appointment time to complete the registration process.
  • Returning Patients: Please arrive 5 minutes prior to your scheduled appointment to check-in. While we make every effort to accommodate our patients, late arrivals may result in longer wait times, or you may reschedule your appointment.

If you are unable to keep your appointment, please give us a 24-hour notice so that we can accommodate other patients.

Any patient who does not notify us of their inability to keep an appointment will incur a $20 rescheduling fee. The practice reserves the right to dismiss patients for repeated no-shows. By scheduling an appointment, you are agreeing to our cancellation policy.

Children

For the safety and security of our young patients, it is our policy that all minor patients under the age of 18 must be accompanied by one parent or legal guardian. If another adult must bring the child to the appointment, please provide written permission for this adult to accompany the patient and to make any necessary medical decisions on behalf of the patient. Until the COVID pandemic is over, additional family members will need to wait in their vehicle or remain at home.
